When an item says Pack of 10 and the price is .49 a piece...does that mean .49 for all 10 or would it be 4.90?

It depends on the item. Some items come with multiple pieces in a pack. You can see if that's the case by the picture. For example, if we have a pack of 6 toothbrushes for 79 cents, you get 6 toothbrushes for 79 cents, and if you order 10 of the item, you get 60 toothbrushes for $ 7.90.
If you can tell me the item number you're looking at, I can tell you for sure how the pricing works.
